DA Form 4949 (Administrative Adjustment Report (AAR)) and DA Form 5692-R Ammunition Consumption Certificate

DA PAM 700-16, para. 12-15a, states DA Form 4949 is used on all ammo, missiles and explosive items along with applicable MFDRs.

​​Is a DA Form 4949 the only form required for expenditure of operational load items?  Or do I also need to complete a DA Form 5692-R when consuming items such as AT-4s and C-4, which are CIIC I, II for operational load purposes?

Additionally, appendix F, para. F-4c, states items in Table F-2 with a "T" will require additional documentation. 

So which forms do I need to complete when expending CIIC I and II items for operational load purposes?

IAW DA Pam 700-16 DA Form 4949 and Missile Firing Data Report (MFDR) for applicable missile systems are required for expenditures of Operational Load Ammunition. NOTE: Ammunition issued in support of testing is exempt from using DA Form 4949 IAW DA Pam 700-16, 12-15. b.  

DA Form 5692-R is only required for Training Ammunition IAW DA Pam 700-16, 12-10 and 13-7. d.

DA Pam 700-16, Appendix F, F-4. c. refers to Training Ammunition not Operational Load.

Operational Load is not to be consumed in training IAW DA Pam 700-16, 12-1. b.

The issuing Ammunition Supply Point (ASP) may have requirements more stringent than the regulation. Suggest you contact them to determine what they require.
